       The Lifetime Service is a joint Community Children’s Nursing and Psychology Service for Children and young people with life limiting, life threatening conditions, and those who meet Children’s Continuing Care criteria.        

The team is based in Kingswood Hub, South Gloucestershire and currently provides a service across four Integrated care board (ICB) areas, Bristol, North Somerset, and South Gloucestershire, supporting children aged 0-19 and their families. The Children’s Continuing Care Team also provide a small service in B&NES.

       The post holder needs the ability to work flexibly to meet the needs of the service to cover a 7-day period covering 365 days per year if and when the Service requires. This involves both day and night working, night shifts are 22.00-07:00 unless in exceptional circumstances

The purpose of this role is to predominantly work in the continuing care team to undertake clinical shifts usually working alongside the Lifetime support workers (LSW) in the child’s home or school setting. To observe their clinical competencies and undertake annual clinical assessments of the LSW’s to enable them to undertake the clinical interventions to the children and their families who meet Continuing Care Criteria (Bristol, South Gloucestershire, North Somerset, and B&NES).

This role will undertake the line management of LSW’s which includes providing training and assessment to a large team of LSW, having 6 weekly one to one meetings and Performance development reviews as well as supporting attendance reviews in accordance with Sirona’s policies and procedures.

  • • Extensive experience of working as a support worker in a community environment
  • • Experience of working with children with complex health needs
  • • Experience in performing safe and effective clinical skills eg: Tracheostomy care, monitoring and recording vital observations and interpreting results, suctioning, enteral feeding
  • • Excellent communication skills, both verbal and written
  • • Ability to manipulate fine tools, e.g., for Venepuncture and other clinical tasks
  • • Insight into how to evaluate own strengths and development needs, seeking advice where appropriate and the ability to take part in reflective practice, clinical supervision activities and apply research to practice
  • • Knowledge of Clinical governance processes (e.g., audit)
  • • Knowledge of Information governance (e.g., data protection)
  • • Knowledge of Mental capacity, deprivation of liberty and safeguarding children and adults
  • • Knowledge of the Principles of infection control
  • • Knowledge of Equality, diversity, and anti-discriminatory practice
  • • Knowledge of Applying evidence-based practice in line with research
  • • Evidence of up-to-date clinical skills relevant to the position or willingness to undertake
  • • Range of IT skills including electronic communication and clinical records systems
  • • Ability to work unsupervised
  • • Positive approach to change and supporting others through change
  • • Ability to work in partnership with the team and others
