Are you looking to take your pharmacy career to the next level? We are seeking a Band 7 Clinical Pharmacist to join our dynamic, integrated pharmacy team, delivering high quality, patient centred care across community hospitals and primary care.

This is a unique opportunity to make a real difference for patients with complex needs and multiple comorbidities, with a focus on polypharmacy, frailty reviews, and holistic care. You will work closely with multidisciplinary teams, attending ward rounds and community resource team meetings, and contribute to optimising patient outcomes across the care pathway.

Main duties of the job

We offer a role that is varied, challenging, and rewarding – supporting patients, mentoring colleagues, and helping to shape innovative pharmacy services across the region.

This is a full-time (37.5 hours) position based within Central BCU, covering both Conwy and Denbighshire.

Qualifications and/or Knowledge

Essential criteria
  • MPharm degree or equivalent
  • Registered with the General Pharmaceutical council
  • Post Graduate MSc/Diploma in Clinical Pharmacy or equivalent experience and/or qualifications
Desirable criteria
  • Non-medical prescribing qualification
  • Accredited or eligible to be accredited as a HEI Clinical Pharmacy diploma/MSc tutor

Experience

Essential criteria
  • Broad post registration experience
Desirable criteria
  • Experience in specialist clinical area

Aptitude and Abilities

  • Good verbal/written communication skills
  • Demonstrate ability to work within a multidisciplinary team setting
