Trosolwg o'r swydd
- Manage patient appointments and clinic bookings.
- Coordinate patient pathways, support waiting list management.
- Maintain accurate records on EPR and other Trust systems.
- Manage clinic changes, cancellations and diary schedules.
- Ensure patient records are handled in line with Information Governance requirements.
- Contribute to service improvements and provide cross cover within the administration team.

Manage Patient Appointments and Pathways:

Process referrals and ensure patients are booked into the appropriate clinics.

Schedule new and follow-up appointments.

Identify potential delays or pathway breaches and escalate issues appropriately.

Maintain accurate pathway information on Trust systems

Manage clinic cancellations and re-book patients within required timescales.

Ensure patients progress smoothly from referral through to discharge.

Ensure patient information is accurate and up-to-date.

Track and manage health records in compliance with Information Governance requirements.
- Attend service development meetings.
