We are currently looking for an enthusiastic and efficient Discharge Facilitator to work across 7 days. During the current COVID situation we have changed the way we support discharges.
You will be the main point of contact for the MDT for discharge planning. Supporting throughout the patient’s journey at the hospital, working as a team with Site Operations colleagues to ensure appropriate cover. You will work as part of the MDT and work closely with external colleagues such as CCGs and Social Services. Supporting a case management style of working.
You will ensure that all patients who require support to leave hospital are managed and supported. You will ideally have experience in health and social care settings. The role is challenging however very rewarding, there are opportunities for progression within the team.
Informal visits are recommended to gain a greater understanding of the role.
The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
