Job overview

The District Nursing service in Sandwell is looking to recruit a District Nurse Team Leader to manage our Red District Nursing Team based in Rowley Regis. Our District Nursing service is made up of 7 teams across the borough and you will be working alongside fellow Team Leaders, Matrons', Community Matrons, Practice Education Leads alongside the main functions of the team such as Clinical Leads, Staff Nurses', HCA's, Nurse Associates and wider members of the MDT. 

The District Nurse Team Leader will lead the team of 20+ members of staff and be the overall caseload holder of the population in the local area requiring housebound Nursing care. The Team Leader will be a role model for junior members of staff and exert leadership skills whilst striving to maintain a well performing team which achieves against it's KPI targets. It is an exciting time to be joining the District Nursing service with the formation of Town Teams' and the prospect of further developments in the service in the years to come.

This role is full time, with weekend and late shift working on a rota basis. You will require a driving licence with access to a vehicle to be utilised for work purposes. 

Main duties of the job

Lead the District Nursing team to deliver well rounded quality care to a cohort of housebound patients' or to patient's in a clinic setting. You will work closely alongside various members of the MDT on operational and clinical work. You will be managing a team of around 20+ members of staff from Clinical Leads to Healthcare Assistants. You will be the overall caseload holder for around 450 patients'. You will be able to demonstrate core trust values whilst ensuring your team delivers on local, trust wide and national initiatives and KPI's.

Detailed job description and main responsibilities

The District Nurse Team Leader manages a large team of individuals including Healthcare Assistants, Nurse Associates, Community Staff Nurses and Clinical Leads to ensure the smooth running of a busy Nursing team focused on providing Nursing care to individuals in their own homes or in a care home. You will be a role model for your team, drive high standards of care, be the overall caseload holder and be responsible to drive improvements through the team and wider service. You will be receptive to change and ambitious to ensure the District Nursing service is equipped for the future. You will be involved in operational decision making including roster control and budget management. You will work alongside other members of a multi-disciplinary team including tissue viability, GP's, Community Matrons and the wider Integrate Community Services directorate and PCCT group.

Qualifications

Essential criteria
  • Registered Nurse with Degree/Diploma
  • V150 Nurse Prescribing
  • Holds a recognised leadership qualification or willing to work towards

Experience

Essential criteria
  • Experience of working in the Community Nursing service at a senior level (Clinical Lead and above)
  • Experience of working individually and as part of a team.
  • Demonstrate the ability to manage a team
Desirable criteria
  • V300 Independent Prescribing
  • Specialist Practitioner Community Nurse Qualification
