Your Health Partnership is a cutting edge Primary Care Network working as part of Sandwell and West Birmingham Hospitals Trust. We are seeking to recruit an independent prescribing senior clinical pharmacist to join our expanding Medicines Management Team in a management role. This exciting opportunity will mean working alongside other members of the primary care team to support medicine optimisation within our extensive and diverse network of GP surgeries.

Main duties of the job

A key role will be involved in developing a strong relationship with our local care homes and through activities such as patient facing structured medication reviews, outlined in the NHS Network Contract Direct Enhanced Service document.  Clinical pharmacists are also key to improving access for out patients, as 20% of our calls to the PCN are related to prescription or medication queries.  The potential to improve patient care and experience in this area is significant - we want you to help shape this change!

The successful applicant will need to share the same high quality values of Your Health Partnership Primary Care Network and work as part of a team which includes GPs, Advanced Nurse Practitioners, Nurse, Physician Associates, and Social Prescribers.

Person specification

Experience

Essential criteria
  • Substantial experience post registration in primary, community or secondary care pharmacy
  • Significant post qualifying experience as a pharmacist
  • Experience operating as an Independent Prescriber in a range of clinical areas
  • Experience and an awareness of the breadth of common acute and long term conditions that are likely to be seen in General Practice

Qualifications

Essential criteria
  • Masters degree in Pharmacy (or equivalent) + Pre-registration year and Specialist knowledge gained through post graduate diploma in clinical pharmacy/therapeutics or equivalent level of knowledge/qualification
  • Registered member of General Pharmaceutical Council (GPhC)
  • Registered Independent Prescriber
Desirable criteria
  • Membership pf the Primary Care Pharmacy Association
  • Membership of the Royal Pharmaceutical Society

Communication/Verbal Skills

Essential criteria
  • Excellent written and verbal communication skills
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety

Knowledge

Essential criteria
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-- based healthcare.
  • An appreciation of the nature of GPs and general practices.
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Excellent IT skills
  • Able to obtain and analyse complex technical information
  • Able to work under pressure and to meet deadlines
  • Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers
  • Safeguarding and other mandatory training
