Walsall Healthcare NHS Trust provides integrated acute and community Healthcare in the West Midlands serving a population of 260,000 residents. Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading. Construction of the new integrated critical care unit was completed in 2018, extension of a Neonatal Unit and an approved plan to build a brand new, state of the art Emergency Department which commenced in 2021

Detailed job description and main responsibilities

To be responsible for the triage / assessment of care needs and the development, of programmes of care, within Scope of Professional Practice.
Demonstrate high level of judgemental/analytical skills when interpreting and performing patient assessments and care planning via the telephone and face to face.
Maintain accurate and up to date patient care records, maintaining patient confidentiality, and statistical records as required by the service.
Work with other professionals to promote multidisciplinary / multiagency working.
Gain access to enhanced programmes of care in accordance with Walsall Healthcare NHS Trusts integrated care pathways
