Trosolwg o'r swydd

Wolverhampton Community Rehabilitation Service is looking for a dynamic band 6 physiotherapist. You will need excellent communication and prioritisation skills with a flexible person-centred approach to join our supportive team.

This exciting opportunity involves working across the hospital to community interface, including the option of accessing a wide range of rotations supporting professional development.

Our community team includes Physiotherapists, Occupational Therapists, Rehabilitation Assistants, Senior support workers and Therapy Technicians. The service covers the Community Intermediate Care Team (CICT), Rapid Intervention Therapy (RIT), Falls Prevention and Community OT and Physiotherapy.

RIT therapy enables same day access to therapy to prevent hospital admission and involves joint working with urgent care advanced clinical practitioners, nurses, and paramedics with access to consultant support.

CICT provides short term (up to six weeks) rehabilitation and re-enablement programmes to patients in their own homes following hospital discharge, illness or injury.

This role involves working extended hours 8am to 8pm - 7-day working pattern.

A full driving licence and access to a vehicle that can be used for work is essential.

Prif ddyletswyddau'r swydd

To complete physiotherapy assessments to in the community preventing admission to hospital, and providing  community rehabilitation. This will include treatment planning, assessing risk, promoting and enabling independent functioning.

To provide training, supervision and advice to students, band 5 therapists, rehabilitation assistants and therapy technicians.

To participate in the planning, development and evaluation of  community therapy service, holding responsibility for defined projects.

To maintain close working relationships with other health professionals /referrers to ensure efficient, effective and appropriate delivery of the service.
