An exciting opportunity has arisen for a registered nurse or allied health professional with experience in clinical leadership to join our Funded Care Team within BNSSG ICB.
This busy and interesting role will give you an opportunity to grow your professional leadership skills in an emerging service. You will join an existing team of clinical leads, providing knowledge, experience and support to a large team of nurses and allied health professionals involved in assessing and reviewing clients with regards to their health needs, welfare and provision of care under the National Framework for Continuing Healthcare.
Our team is based in the heart of Bristol operating a hybrid working model. Staff are required to attend the office base regularly, travel to meet patients in their home environment and our also supported to work from home. Using this hybrid model, we are flexible to meet the needs of both our staff and service, whilst maintaining support to the wider health and social care network. To deliver this service we use a range of technology, so the post holder will be required to be IT literate and able to navigate various software and remote IT platforms.

Main duties of the job
The chosen candidate will have a strong ability to review evidence and provide clear rationales for decisions, be able to problem solve and develop these skills in others. They will be confident in managing challenging situations, setting expectations of available options for discharge in line with ICB policies and processes. They will lead a team of staff, maintaining good morale and act as a role model of professionalism, engaging in service development and change management with a positive outlook and determination to achieve agreed objectives.
The job holder will be involved in directing care coordination as well as identifying risk, service improvement and governance issues. An ability to work collaboratively with other agencies is essential and experience of working in such a manner is desirable. As this role may involve working with clients within other areas of care, such as learning disabilities or mental health, a broad range of knowledge and experience is welcomed, however, training will be tailored to your experience and the needs of the service.

Detailed job description and main responsibilities
You will develop your critical thinking and decision-making skills, weighing up clinical need against resources, safety and patient choice. You will need a sound knowledge of the Mental Capacity Act and Best Interest Decisions and be able to think laterally to meet patient need.
You will provide leadership and support to the complex case part of the team, specifically within the area of flow and complex discharges. You will be responsible for supporting flow of individuals through the pathway 3 process, including enhanced care reviews and Continuing Healthcare (CHC) assessment, supporting discharges of our most complex individuals and leading our neurorehabilitation process.
Person specification
Knowledge, Training and Experience
Essential criteria
- Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area
- Knowledge of application of National Framework such as CHC and FNC
- Evidence of continued professional development
- Experience of managing risks and reporting
- Understanding of the public sector
- Demonstrated experience in a Healthcare environment
Communication Skills

- Adapt and respond to the communication requirements of a diverse range of patients/clients. Conversations may be challenging /emotional.
- Using effective communication skills to influence negotiate and facilitate to build effective working relationships with internal and external colleagues/managers.
Analytical

- Ability to analyse very complex issues where material is conflicting and drawn from multiple sources.
- Demonstrated capability to act upon incomplete information, using experience to make inferences and decision making
- Experience of setting up and implementing internal processes and procedures
Planning Skills
Essential criteria
- Demonstrated capability to plan over short, medium and long-term timeframes and adjust plans and resource requirements accordingly
- Ability to analyse very complex issues where material is conflicting and drawn from multiple sources.
