Job overview
To work as a member of the Amersham Rapid Response & Intermediate Care team doing nursing, social and rehabilitative care and assessment for designated patients (either individuals or a specific clinical group.) The overall aims of the Rapid Response & Intermediate Care Team are to avoid inappropriate admission into the Acute Service and facilitate timely discharge into the community.
Main duties of the job

Your main duties include:
- Providing general and specific care to adults within competencies
- Being an integral member of the Rapid Response & Intermediate Care (RRIC) team, providing defined delivery of nursing, social and rehabilitative care to patients in liaison with senior staff.
- Taking responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients’ own homes.
- Being responsible for the completion and potential delegation of non-clinical tasks within the team.
Your main responsibility is to :
- Improve the care of patients in the community, so patients can remain safely in their own home for as long as possible
- Have an innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery
- Willingness to further develop your clinical expertise in a range of areas
A full UK driving licence and access to a car for work purposes

- Previous caring experience of working in Health or Social Care environment.
- Awareness of the needs of older people.
Desirable criteria
- Awareness of the needs of people with long term conditions.
- Previous NHS/Social Care/ Voluntary Organisation experience.
Special circumstances
Essential criteria
- Physically able to perform task whilst kneeling, bending and able to handle patients and equipment safely.
- Valid and current driving licence and car to be able to travel to locations throughout Buckinghamshire
- Ability to show patience and dependability

SKILLS, ABILITES & KNOWLEDGE
Essential criteria
- Previous caring experience of working in Health or Social Care environment.

EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
- Good literary and numeracy skills.
- Health and Social Care Level 2 or 3 Diploma or equivalent relevant patient facing experience and willingness to undertake Level 3 qualification.
- Have the ability to undertake training to acquire the skills appropriate for development in the post
