Job overview

An exciting opportunity has arisen to join the Night service Community Team at Buckinghamshire Healthcare NHS Trust 

Do you want the privilege of caring for patients in their own homes to deliver increasingly complex, holistic care? If yes, community nursing may be the right career choice for you. We are able to offer part-time roles along with a full skills development and preceptorship programme.

We are ideally looking for someone who can work autonomously in the community at Community Staff Nurse level. As a community nurse, you will work alongside a Case manager and a Community Healthcare Assistant, but at times will take the lead in triage and making care decisions; nursing patients with multiple complex health care needs in their own home. 

Main duties of the job

To contribute to processes for service and quality improvement
Assess the physical and psycho–social needs of patients and deliver 
therapeutic interventions based on best available evidence in order to 
improve health outcomes.
 To manage own case load, delivering and evaluating service delivery to 
patients with a variety of medical conditions and those delegated by case 
managers. 
 To plan interventions with the Case Manager and other team members 
using the common assessment process
 To take part in the referral management process to ensure patients are 
treated by the most appropriate member of the team/service
 To contribute to the development of protocols and procedures developed 
for clinical pathways 
Assist in the development of case management protocols and guidelines 
within their own area of professional practice

 Participate in weekend rotas / 7 day working rotas

Education & Qualifications

Essential criteria
  • Registered health professional - Diploma level
  • Evidence of recent CPD
Desirable criteria
  • Post graduate module(s) in long term conditions

Skills, Abilities & Knowledge

Essential criteria
  • Demonstrate profession specific clinical assessment planning delivery and evaluation skills
  • Able to work under pressure
  • Can work independently, for example, to manage catheter care, end of life care (including verification of death, syringe driver management, psychological support and symptom control), wound care, diabetic management, prevention of admission
  • Is willing and able to develop further skills to support the community team
Desirable criteria
  • Knowledge of modernisation agenda within the NHS and social care
  • Good written and verbal communication skills
  • Report writing skills
  • IT skills
  • Presentation skills

Experience

Essential criteria
  • Demonstrate experience post or pre-registration placements in relevant clinical areas: palliative care experience; catheterisation experience
  • At least one year community experience
  • Is willing and able to undertake night time driving and drive a large SUV
Desirable criteria
  • Knowledge of working with common assessment process
  • Experience of electronic clinical systems
  • Experience participating in a research or audit project

Special Circumstances

Desirable criteria
  • Car driver / owner able to travel to other locations within the trust
