Job overview
This is a system-wide post with a focus on provision of on-site clinical and professional support to partners across the footprint of North Cambridgeshire & Peterborough Care Partnership - Fenland, Huntingdon and Peterborough. CUH is the host organisation and the employer. The system Consultant/Lead Pharmacist – Older People, also hosted by CUH, is the line manager. The purpose of the post is to:
- Support the Consultant Pharmacist in developing and delivering a strategy for care models that identify and manage older patients at greatest risk of severe medicine-related adverse effects.
- Be accountable for the delivery of delegated workstreams in line with system and service priorities
- Act as an expert resource in medicines optimisation in older people, liaising with the multidisciplinary team and system partners to promote safe, effective use of medicines in this patient group.
- Provide an advanced clinical pharmacy service to specific cohorts of older people.
- Collaborate with pharmacy teams from other providers and primary care to ensure service delivery to agreed standards in line with business planning requirements and NHS contracts e.g. DES
Main duties of the job

CUH is the host organisation and the employer. The system Consultant/Lead Pharmacist – Older People, also hosted by CUH, is the line manager. Honorary contracts will be facilitated to enable delivery of direct patient care to patients under the care of partner organisations where necessary e.g. North West Anglia NHS Foundation Trust (NWAFT) and Cambridgeshire and Peterborough NHS Foundation Trust (CPFT).

Qualifications
Essential criteria
- 1. Masters level degree in pharmacy.
- 2. Registered as a pharmacist with General Pharmaceutical Council (GPhC).
- 3. Diploma in Clinical Pharmacy/General Pharmacy Practice (or equivalent) *.
- 4. Registered as an independent prescriber with the GPhC.
Desirable criteria
- 1. MSc in Clinical Pharmacy (or equivalent qualification)*.
- 2. Leadership or management development course.
- 3. Risk management training.
- 4. Educational Qualification
- 5. Member of Royal College of Pharmacy
- 6. Research degree (MSc,MRes, PhD) or working towards this qualification.
Experience

- 1. Extensive recent post-registration experience as a clinical pharmacist (hospital or primary care)_ at band 8a (or equivalent) in the specialist clinical area
- 2. Recent significant experience of patient consultation.
- 3. Recent practical experience of independent prescribing in the specialist clinical area.
- 4. Highly specialised knowledge of drugs and therapeutic trends in specialist clinical area.
- 5. Experience of the practical implementation of national guidance in drug use (e.g. NICE).
- 6. Experience of documenting pharmaceutical care issues in patient records.
- 7. Experience of writing professional reports.
- 8. Experience of developing and reviewing patient group directions and shared care guidelines, drug policies and treatment guidelines.
- 9. Evidence of having written, supervised, conducted and implemented the outcomes of clinical audit.
- 10. Experience of the practical application of antimicrobial stewardship initiatives.
- 11. Experience of drug expenditure analysis and reporting.
- 13. Experience of the management of high-cost and high-risk medicines.
