Job overview
The post holder will assist in the coordination and delivery of the Clozapine clinic and a physical health strategy for locality service users including the assessment of physical health through a clinic setting and within an individual’s own home environment where needed. This will include regular monitoring of physical health measures, healthy lifestyle advice and to ensure access to physical health interventions and deliver them where appropriate to do so.
The post holder will be an integral member of the Clozapine clinic team and will deputise for the clinic lead and be a senior on site presence.
You will liaise with GP's and other health professionals where indicated to deliver holistic care and a complete client health record to ensure a quality and integrated service is delivered.
Main duties of the job
- To ensure the delivery of the service within the Clozapine clinic is in accordance with CPFT Clozapine operational policies.
- To ensure the delivery of the service within the Physical Health clinic is in line with CPFT Medicines Management policies.
- To attend agreed meetings, meet supervisor and team manager on matters of service development.

Detailed job description and main responsibilities

• To provide professional leadership for staff within the CALT on service user assessment and care planning of physical health and well being.
• To have good evidence-based knowledge of the physical health needs of locality team service users.
• To have good evidence based knowledge around the use of clozapine and to support the service and staff in clozapine initiation within the home environment.
• To support the administration of long acting antipsychotic depots in clinic and other environments and in particular the management of Olanzapine depots within the clinic setting
• To support monitoring of lithium levels .
• Develop strategy for those service users not able to engage in the clinic model to receive physical health checks.
• To lead, deliver and oversee the smooth running of physical health clinic
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Education / Qualifications
Essential criteria
- Registered Mental Health Nurse
- or General Nurse (RGN) with interest in mental health
- Evidence of further training appropriate to physical health management and interventions or willingness to undertake
Desirable criteria
- Evidence of continued training and professional development
- Evidence of training appropriate to adult mental health care
- Evidence of training appropriate to physical health management and interventions
- Health coaching qualification
Experience
Essential criteria
- Experience of working with adults with psychosis
- Experience of working independently
- Experience in undertaking physical health checks
Desirable criteria
- Experience of Clinical Supervision experience
- Experience of working with individuals, families, multidisciplinary teams and community services
- Lived experience of mental health
Skills and abilities
Essential criteria
- Compentent in undertaking a range of physical health checks including blood taking, ECG, BP, Sexual Health screening
- Ability to work autonomously
- Knowledge base around suitable interventions to address identified health issues
- Able to develop good therapeutic relationships with clients
- Planning and organisational skills
- Confident and competent in delivering educational sessions
- Able and confident to communicate well with a variety of people including GP’s , practice nurses, health specialists, pharmacists and other key stakeholders

- Knowledge pertaining to physical health checks and baseline measures and knowing when further action is indicated.
- Knowledge of monitoring, side effects and effective clinical management of Clozapine
- Knowledge around addressing identified health issues and of community resources and teams to partner with
- Knowledge of contemporary policy and legislation relating to mental health and social care
- Ability to identify and employ, as appropriate clinical governance mechanisms for the support and maintenance of effective clinical practice
- Demonstrate a knowledge of the issues facing service users including social, financial and health issues
Physical requirements
Essential criteria
- Physically be able to undertake a demanding role safely, as and when required
- Ability to travel independently around the Cambridgeshire and Peterborough area meeting required timescales. A full driving licence and access to a vehicle is essential.
