Job overview
The Band 5 Development CHC Assessor is an entry-level practitioner role designed to build the skills, confidence, and knowledge needed to become a fully autonomous CHC Assessor.
This role forms part of a deliberate “grow our own” approach, giving developing clinicians the structured support, supervision, and learning opportunities they need to progress within the Continuing Healthcare pathway.
The postholder works under the guidance of experienced CHC Assessors and Senior Assessors, taking on aspects of the CHC assessment and review process in a gradual, supported way.
They contribute to evidence gathering, liaise with professionals, assist with completion of Decision Support Tools, and learn how to interpret the National Framework in real-world practice. As their competency develops, they will take responsibility for more complex tasks and increasingly work independently.
A key part of the role is active learning. The Band 5 practitioner will participate in formal training, reflective supervision, competency-based development, and regular feedback to build strong foundations in assessment practice, clinical reasoning, communication, and person-centred decision-making.
They are expected to approach the role with curiosity, openness, and a willingness to grow. The Development Assessor plays a valuable role in supporting the team while preparing for future progression into a Band 6 role.
Main duties of the job
The purpose includes:
• To provide effective coordination and administrative support to services, projects, and programmes. Supervise assistants as required.
• To coordinate meetings, communication, and information flow across teams, where some information may be sensitive, complex or contentious.
• To assist in monitoring performance and progress against agreed objectives, escalating issues where necessary.
• To contribute to improvement activity by collecting and analysing information, identifying issues, and supporting delivery of solutions.
• To build strong working relationships across teams and partners to support joined-up, efficient, and high-quality service delivery.
