Main duties of the job
Reporting to the Trafford Ward Manager, you will assist the team to deliver personal care to our patients who have a variety of rehabilitation and End of Life care needs for Cambridgeshire and Peterborough NHS Foundation Trust Working with a team of passionate and dedicated Health Care professionals, you will be responsible for delivering a high level of care to the patients in our community wards This post is open to professionals with the right attributes to enhance a team. This post involves rotational shifts across 24 hours
- Ensure the delivery of high quality, patient focused care and treatment which promotes the “Essence of Care” and “Standards for Better Health” principles.
- Ensure Expected Date of Discharge (EDD) is set through negotiation between patient, carer, team and internal/external agencies to achieve a safe and effective discharge which closely meets the patient’s needs.
- Communicate effectively with patients and their families /carers'
- Ensure delivery of effective patient centered care programmes', through appropriate leadership,
- ensure monitoring of quality and compliance with standards is demonstrable within the service on an ongoing basis.
- To maintain a safe and healthy environment for patients/ clients, visitors and staff

Detailed job description and main responsibilities

- To undertake physical health checks including venepuncture, and other tasks as required.
- To work across inpatient units if required
- Support the nurse in charge in inpatient units in maintaining essential clinical stocks.
- To promote a positive service user experience throughout their stay through proactive listening, being responsive to issues raised and through resolving issues and concerns raised by service users with colleagues and senior members of the team.
- To be responsible for maintaining and conducting oneself in a professional manner towards service users, carers, colleagues and other agencies, at all times.
- To participate in working collaboratively with other Trust services and external agencies to provide seamless care for service users.
- To observe and report to a registered member of the team, any changes which may indicate an alteration in the general well being and safety of a service user. This would include observations around the service user’s presentation, behaviour, mood and verbal content.
- To provide service users with personal care, where and when required, and demonstrate empathy, dignity, respect and compassion at all times.
- Ensuring that any entries made by the post holder in the nursing notes are current, relevant and accurate, and the content will then be reported & counter signed by the registered nurse or relevant professional.
- Working in partnership with the service user to access a range of activities and services to maintain their independence. Promoting healthy activities and lifestyle choices.
- Ensure that the ward environment is clean and tidy at all times. To liaise with the ward housekeeper regarding cleanliness.
- To promote and support registered staff with the physical health, comfort and wellbeing of service users, including physical observations and taking blood pressure, temperature and pulse under their guidance and supervision.

Education/Qualifications
Essential criteria
- RN, Diploma of Education (Adult Nursing) or similar evidence of ongoing updating
Desirable criteria
- NVQ D32/D33 Award
Experience
Essential criteria
- Experience of working with older people
- Co-ordination of a team of care staff
Desirable criteria
- Work in a Rehabilitation setting
Skills & Abilities
Essential criteria
- Good range of practical nursing skills appropriate for the ward
- Understanding of multidisciplinary team working
- Knowledge of elderly care and the principles of rehabilitation care
- Excellent communication skills
- Effective time management
- Basic Computer Skills
- Principles of Planning for Discharge
- Able to work under own initiative and direct the work of others
- Has a range of mechanisms for dealing with stress, can recognise when to use them and does so
- Can demonstrate flexibility of approach
- Ability to work flexibly to meet the needs of the client group
- Able to demonstrate commitment to high quality care and service provision

You must have appropriate UK professional registration.
