Job overview

An exciting opportunity has arisen for a Band 6 Diabetes Technology Educator to join the new Hywel Dda Health Board wide Insulin Pump Team 

Applicants must be either registered with the NMC as  an adult Nurse or have HCPC Registration.

This is a full-time post however part time will be considered

The post holder will be responsible for the management and delivery of diabetes education to patients and staff across Hywel Dda Health Board with links to Primary Care, external services and voluntary organisations.

Main duties of the job

The successful candidate must be able to :

  • Implement, organise, and deliver programmes of evidenced-based diabetes education for staff and patients with and without an insulin 
    pump, and evaluate programmes of education for people with diabetes. This will include pregnancy and patients transitioning to the adult diabetes pump service on insulin pump.
  • Work with the multi-disciplinary team (MDT) to provide education and support for those patients and their carers using intensive insulin therapy and/or diabetes technologies - insulin pumps; continuous glucose monitoring (CGM); Hybrid Closed loops (HCL) in line with National Institute of Clinical Evidence (NICE) / Joint British Diabetes Societies (JBDS), Dose Adjusted For Normal Eating (DAFNE) and Diabetes Technology Network (DTN) guidelines

 

  • The post holder will be guided by Diabetes Specialist Nurses/Dietitians in education of insulin pump therapy including starting patients on insulin pumps, continuous glucose monitoring. Providing advice and support to patients already using an insulin pump. 

• Develop and deliver patient and staff education programme in diabetes technology aligned with current guidance 

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications and Knowledge

Essential criteria
  • NMC Adult Registered Nurse / HCPC Registrant Practitioner with Degree supplemented by post graduate diploma specialist training in relevant area, experience, short courses
  • Demonstrate knowledge of management of type 1 diabetes; carbohydrate counting; insulin dose adjustment, insulin pumps, continuous glucose monitoring and hybrid closed loops.
  • IT competent with experience of using Microsoft packages/ digital platforms
  • Experience of facilitating and delivering teaching sessions both face to face and digitally
  • Ability to work flexibly to meet the needs of the service and to work across all sites as required
Desirable criteria
  • Mentorship Course / Teaching and Assessing qualification
  • Health Promotion qualification or skills
  • Counselling qualification / Motivational Interviewing
  • Quality Improvement experience

Experience

Essential criteria
  • Experience in delivering Structured Education for patient with diabetes
  • Recent diabetes care experience at band 5
  • Ability to work independently and as part of the diabetes team
Desirable criteria
  • Demonstrable experience of audit and/or research
  • Knowledge of evidence-based practice for the management of Type 1 diabetes and insulin pumps.
  • Experience of Dose Adjusted For Normal Eating Educator (DAFNE)
  • Structured patient education (XPERT and XERT Insulin) Educator

Language Skills

Desirable criteria
  • Welsh Speaker (Level 1)
