Job overview

The Team Leader will be responsible for managing an NHS Continuing Healthcare Team as well as co-ordinating the systems and processes applicable to Continuing NHS healthcare. 

Our Long Term Care Specialist Nurses, who used to be known as Nurse Assessors, are also now providing support through a recently developed Long Term Care Pathway which entails supporting secondary care with the Continuing NHS Health Care and NHS Funded Nursing Care processes. This pathway is aimed at ensuring a more smooth and patient focused transition from acute episode, through discharge planning and then Nursing Home placement.

A full understanding of the NHS Continuing Health Care Criteria and knowledge of Funded Nursing Care is desirable although this can be developed following appointment.

Responsibilities will include scrutinising applications for NHS Continuing Healthcare against the criteria for funding, referring back to originator with any queries/issues, attending case/review conferences to discuss new and current packages, managing complex discussions and negotiating resolution as appropriate taking into account both the clinical and financial implications/risks to the client and the Local Health Board, providing daily management to a team of band 6 Long Term Care Specialist Nurses on a daily basis which includes human resource issues and a reporting mechanism to the senior managers.

Are you a first level General Nurse or a Registered Mental Health Nurse with varied experience looking for this new challenge? If so, an exciting opportunity has arisen for the post of Long Term Care Specialist Nurse Team Leader with the Long Term Care Team in Hywel Dda Health Board.

The person we are looking for will be an enthusiastic experienced Nurse who has excellent leadership and communication skills and who enjoys a challenge. We have a team of Long Term Care Specialist Nurses who work closely with a number of other professionals.

Hywel Dda University Health Board is a dynamic and innovative place to be and this role is no different. The Team Leader will be responsible for managing an NHS Continuing Healthcare Team as well as co-ordinating the systems and processes applicable to Continuing NHS healthcare. The Team itself is responsible for the management of the NHS Continuing Health Care criteria on a Health Board wide basis as well as the monitoring of people eligible for NHS CHC in care home environments. The successful post holder will be responsible for assisting the Strategic and Senior Operational lead for NHS CHC, and assisting in policy changes and implementation.

The base can be in either Carmarthen or Llanelli, but may be subject to change. However, you will need at times to be able to travel across the whole of the Hywel Dda area.

Qualifications and Knowledge

Essential criteria
  • Registered healthcare professional
  • Evidence of professional development & life long learning to postgraduate diploma level
  • Degree in appropriate area of clinical practice
  • Postgraduate qualification at Masters level, or equivalent experience
Desirable criteria
  • A management qualification

Experience
