Job overview
This is an exciting opportunity for the successful candidate to form part of a clinical team based in Pembrokeshire which works closely with related services in the specialty, both in the county and throughout the wider Health Board.
We welcome applications for this part time substantive post for a General Practitioner to work in a Specialist Osteoporosis Service. The working pattern of this post are flexible to the needs of the successful applicant, and we are open to discuss a working pattern including either fixed clinical sessions or spreading working hours across different days.
This post will form an integral part of the Pembrokeshire Osteoporosis Service which is an established service that provides specialist care to those with complex osteoporosis.
The Pembrokeshire Osteoporosis Service provides specialist bone health management to the population of Pembrokeshire ranging in age from the late teenage years to the very elderly. We combine the use of both virtual and face to face appointments to best meet the needs of our patients. A fundamental principle of the service is helping to empower patients to make decisions about their bone health management that are right for them.
Main duties of the job
Clinical Responsibilities
The post holder will be responsible for managing their own caseload of new and follow up patients independently, within the framework of regular Consultant input. The service holds weekly MDT meetings for case discussion which serve both a clinical and an education role.
Clinics are provided in both a virtual and face to face manner and a founding principle of the service is to ensure patients are supported to make the best decision for them around their bone health.
The work programme is not fixed and will be discussed with the successful candidate depending on their interests and clinical needs of the department.
The role includes:
1. Providing holistic assessments of bone health, either virtually or in a face to face setting, based on the principle of providing evidence-based care and treatment in collaboration with other members of the Osteoporosis Services.
2. Work in combination with other services and teams across the system to ensure the provision of the best possible care. Examples of associated teams include the Health Board wide FLS, other Specialist Osteoporosis Services in the Health Board, Orthogeriatric Services etc.
3. Aiding in the assessment and coordination of referrals from Primary and Secondary Care colleagues.

· Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Qualifications & Knowledge
Essential criteria
- Full GMC Registration with a licence to practise and entry on the GP Register
- Certificate of Completion of Training (CCT) in General Practice / Certificate Confirming Eligibility for General Practice Registration (CEGPR), or equivalent
- Included on a Medical Performers List or eligible
- Evidence of continuous professional development
- MRCGP
- Conducting sessions in a General Practice Setting
Desirable criteria
- Academic excellence (prizes, merits, distinctions etc
Experience

- Vocational GP training
- 2 years post CCT general practice experience
- Knowledge of UK Hospital systems (or equivalent)
