We have an exciting opportunity for  a Band 5 enthusiastic, motivated, and keen nurse to join our established Rural Alliance  Care Community Team to support and deliver a range of quality nursing services to our patients.

Your skills will be really valued by our teams and patients alike. If you are new to community health, this is a great opportunity to gain experience in this unique field of nursing.

We will do our best to welcome you and help you to settle in as a member of the team. Your contribution, time, skills and caring will make a huge difference to the patients we care for.

This is a part time vacancy 22.5 hours per week on a permanent basis.

Main duties of the job

It’s important that you have access to transport for work purposes. But, of course, mileage expenses are reimbursed in addition to your hourly rate.

As a Community Staff Nurse the successful candidates will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.

You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.

**We offer a relocation package of up to £8,000**

The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

To be eligible for the relocation payment, you must live more than 40 miles from the base/location of the post you are applying for and you would need to relocate to within 15 miles of the new base.

If this applies to you, please state this on your application so we can discuss this with you if you are invited to interview.
