We are seeking a compassionate and dedicated Community Clinical Support Worker to join our community care team. You will support patients in their homes and community settings, helping them maintain independence, dignity, and quality of life. This role is ideal for someone passionate about person-centered care and making a meaningful difference in people’s lives.

We are seeking someone who thrives in a community setting, enjoys building positive relationships, and is passionate about enabling people to live safely at home. Compassion, reliability, and dedication are at the heart of this role.

Hours: 15 Hours per week

Main duties of the job

Assisting qualified nurses to provide safe and effective care in the community for housebound patients and to assist with procedures in the nurse led clinic if required.

Carry out wound care and dressing changes under the direction of a qualified nurse.

Support the nurses responding to crisis visits and other urgent patient needs.

Observe, record and report changes in patients' condition.

Assist with administration tasks as required and maintain an accurate appointment system.

Support patients and their families, offering reassurance and good communication.

Promote  patient independence and patient dignity at all times.

Qualifications

Essential criteria
  • 5 GCSE’s or equivalent, grade C or above. Must include English.
  • Computer literate to ECDL (European Computer Driving Licence) standard or equivalent.

Knowledge and Skills

Essential criteria
  • Computer literacy
  • Awareness of current NHS Priorities and targets
  • Good communicator – able to deal with people at all levels.
  • Ability to handle sensitive information without compromising confidentiality or trust.
Desirable criteria
  • Medical terminology Knowledge of NHS Computer Systems and experience with Patient Management System and Emis system
