Are you a motivated and experienced registered health professional with a passion for personalised care and improving patient outcomes?

Cheshire and Merseyside Integrated Care Board (ICB) is seeking two dynamic and enthusiastic Band 6 Senior Individual Assessment and Commissioning (IAC) Personal Health Budget Nurses/Practitioners to join our growing team. We are recruiting for:

  • One post covering Merseyside
  • One post covering Cheshire

This is an exciting opportunity to play a key role in supporting individuals to have greater choice and control over how their healthcare needs are met through the effective delivery of Personal Health Budgets (PHBs).

Main duties of the job

Working within the Individual Assessment and Commissioning Team, you will collaborate with individuals, families, carers, healthcare professionals and partner organisations to develop innovative, person-centred care solutions. You will provide clinical leadership and expertise in the assessment, implementation and review of Personal Health Budgets, ensuring care plans are safe, effective and aligned to individual health and wellbeing outcomes. Supporting individuals and families to develop personalised care and support plans.

  • Assessing clinical needs and reviewing care packages funded through Personal Health Budgets.
  • Managing a caseload of PHB recipients.
  • Working collaboratively with multidisciplinary teams, providers and local authority colleagues.
  • Providing advice, training and support on Personal Health Budgets across the system.
  • Contributing to service development and the expansion of personalised care initiatives.

Person specification

Qualifications

Essential criteria
  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
  • Registered Nurse/Social worker/HCP with an active UK registration
  • Evidence of post qualifying and continuing professional development

Knowledge

Essential criteria
  • Knowledge and expertise of National Framework for NHS Continuing Healthcare and funded nursing care and related personal Health budget and direct payment guidance/regulation /policies
  • Working knowledge of the personalisation and SEND agenda
  • Working knowledgeable of Who Pays Guidance 2020.

Experience

Essential criteria
  • Skilled at undertaking assessments and ability to make decisions based on assessments and to justify decisions to professionals and cares based on evidence.
  • Demonstrated experience in a Healthcare environment
  • Demonstrate experience and understanding of the concepts of Adult Safeguarding including the Mental Capacity Act.
  • Conversant with Mental Health Act 1983 and Care Act assessment Able to investigate complaints and presentation to the Courts
  • Previous experience in similar role in public sector

Communication

Essential criteria
  • Clear communicator with excellent writing, report writing and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences
  • Skills for communication on complex matters and difficult situations, requiring persuasion and influence.

Analytical

Essential criteria
  • Ability to analyse and interpret information, pre-empt and evaluate issues, and recommend and appropriate course of action to address the issues
  • Problem solving skills and ability to respond to sudden unexpected demands
  • Independent thinker with demonstrated good judgement, problem-solving and analytical skills
  • Takes decisions on difficult and contentious issues where they may be a number of courses of action.
