Job overview
Main duties of the job
In this role you will be responsible for;
-
Providing vulnerable families personalised antenatal care, support and education to enhance their care and improve outcomes
-
Providing vulnerable families with enhanced postnatal care up until discharge from community maternity services at 28 days after birth
-
Working within the specialist community midwifery team (Indigo Team) to implement personalised support plans
- Working as part of the multi-agency team around the family including through safeguarding procedures
-
Signposting to community and specialist services and working within the wider multidisciplinary agencies and VCSE organisations

Working flexibly in the community setting
-
Advocating for women both within maternity services and the wider community including by identifying and addressing barriers to engagement

Promoting physical wellbeing, such as providing aqua natal classes and walking groups
- Delivering essential baby items and equipment sourced via local baby banks

Data collection and evaluation of the role throughout its implementation

Driving and access to own car is essential for this role

- Experience of working in health or social care.
- Experience of undertaking and recording clinical observations and escalation of changes or findings.
Desirable criteria
- Experience of working in an NHS care role.
Education & Qualifications
Essential criteria
- Numeracy and Literacy Level 2
- Care certificate
- Level 3 vocational qualification or Short courses and completion of competency workbook equivalent to Level 3 in a range of patient and clinical care duties and/or Equivalent knowledge and experience in a range of clinical care duties equivalent to Level 3; - Theoretical knowledge - Experience of a range of care and related procedures - clinical observations - knowledge of relevant legislation
