Hospital at Home service provides hospital level care for patients in their own home to avoid hospital admission and facilitate discharge. Urgent community response reviews patients within 2 hours of referral to avoid hospital admission.
Hospital at Home offers a hospital standard of community care in people’s own homes for frailty patients – who are at highest risk of poor health outcomes such as falls, disability, admission to hospital, or the need for long-term care.
The service helps to reduce avoidable hospital admissions, while ensuring patients get the care they need from the comfort of their own homes.
Patients are monitored daily by a multidisciplinary team and are overseen by a GP or consultant. Visits from the team are arranged as and when required to meet individual needs.
