Job overview
The East Home First Team are looking for a Community Support Worker to provide a versatile and responsive service to ensure that opportunities for patients’ reablement are optimised within the community setting.
The reablement workers within each locality will form a part of an Integrated Care Pathway for older patients that involve a range of services to prevent hospital admission and support discharge from hospital.

Main duties of the job
The post holder will deliver rehabilitation support and care to patients in their own home; this may also include Residential homes and nursing homes
The post holder will be flexible in working patterns to meet the needs of the service and will be rostered to provide the required service.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use out services.

- NVQ Level 3 qualification in care or equivalent experience
- Evidence of experience in care setting

- Additional relevant training in Healthcare
