Job overview

 To provide a clinical pharmacy service to the Respiratory Speciality within the Medicine directorate at RCHT.

 

This is a progression post, band 6-7

Main duties of the job

·       Providing core clinical pharmacy service to respiratory inpatients in line with the Trust and clinical pharmacy standards.

 

·       Ensuring high quality prescribing and care is provided on and during admission through to discharge to respiratory patients by effective clinical screening. Ensuring that pharmaceutical knowledge and skills are applied through medication reconciliation on admission, optimisation during their admission and input to planning for a successful discharge process where appropriate.

 

·       Identify medicine-related causes for admission and develop treatment plans to optimise care.

  • - Degree in Pharmacy
  • - GPhC Registration
  • - Clinical diploma or equivalent or working towards
Desirable criteria
  • - Non-medical prescriber
  • - RPS membership

Experience

Essential criteria
  • - Significant post registration covering a range of specialities
  • - Sufficient experience as a clinical pharmacist
  • - Clinical supervision or training of junior staff
Desirable criteria
  • experience of providing a clinical pharmacy service in a hospital environment
  • - Experience of deputising for a more senior clinical pharmacist
