Want to join a multi award winning Heart Failure and Cardiology Team?
We have an exciting opportunity to join the Community Heart Failure Team as Advanced Clinical Practitioner (ACP). This post will be based in the community, supporting patients across Morecambe Bay to have timely access to Heart Failure and Cardiology.
The post holder will hold a clinical caseload of more complex patients, provide clinical leadership to the team via clinical supervision and our in-house CPD programme.
The post holder will work collaboratively with the Clinical Service Manager to implement our Clinical Strategy and contribute to the wider Heart Failure Improvement plan with our medical and nursing colleagues.
Applications are invited from candidates who are Nurses and have an MSc in Advanced Clinical Practice or working towards this award.
We also will consider applicants who are willing to undertake the MSc programme. During this period of training, the post-holder will be paid under Annex 21 of AFC
Main duties of the job
- Act as a practice leader, proactively manage own caseload and work with partners across professional, organisational and system boundaries to improve Heart Failure and Cardiology Services
- Provide consultancy services regarding therapeutic interventions, practice and service issues within Heart Failure and Cardiology Services.
- Within budgetary constraints review and prescribe medications, and lead on prescribing issues within Heart Failure and Cardiology
- Perform advanced clinical skills in the assessment of patients including taking a clinical history, performing a clinical examination, ordering and interpreting diagnostic tests and using advanced clinical reasoning to produce an appropriate treatment plan.
- Acts at all times within the NMC’s Code of Conduct, working within clinical and professional protocols and guidelines.
Working for our organisation
We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

To deliver high quality, patient focussed, evidence based care to patients with acute and chronic Heart Failure and Cardiology issues.
To support the Heart Failure and Cardiology Team in delivering care services to the population of Morecambe Bay. These services include (but are not exclusive):
- Heart Failure Clinics and Home Visits
- Arrhythmia Clinics
- Colleague and system wide training
