Community stroke nurse

Are you interested in working with stroke survivors to support them in their recovery? 

We have an exciting opportunity arising for a full-time community stroke nurse to join our team working with stroke survivors, their families and carers following discharge from hospital. An initial pilot of the service demonstrated benefits to patients and there is on-going work looking at long-term funding for the service. The post is a temporary fixed-term contract until the end of March 2026 while this work is undertaken. 

The community stroke team works with patients, their families, carers and hospital staff to facilitate a smooth transition from hospital to discharge home in the community. The team works with the more complex, disabled stroke patients to ensure they receive the support and care during this process and up to 8 weeks following discharge home. We are a small team of nurses, occupational therapists, case manger, psychology and admin. The work is varied, stimulating and very rewarding. 

The base can be negotiated at interview but the post holder will cover East North Cumbria catchment areas including Carlisle, Longtown & Brampton, Wigton, Solway and Eden.

  • Working closely with other MDT members with an inter-disciplinary approach to patient centred care. 
  • Working autonomously to provide holistic patient centred assessments, treatment planning and evidence based interventions to patients following a stroke, in their own home. This includes working with those discharged into step-down rehab or care placements and care homes. 
  • To work collaboratively across the North Cumbria Integrated Stroke Service to ensure a seamless provision of stroke services across the acute hospital, inpatient rehabilitation and community service pathway. 
  • To supervise and support staff and students 
  • Contribute to service development and data collection within North Cumbria Integrated Stroke Services. 
  • The role involves driving in the community to provide patient care in the NCIC catchment area.  The team does utilise virtual service delivery options when appropriate and there is the opportunity of working flexibly between base, community and homeworking.
