An exciting opportunity has arisen to appoint an experienced and enthusiastic individual as a Clinical Leader within The Transfer of Care Hub 

 

Main duties of the job

You will be able to:

You must be highly motivated, willing to take on new challenges and prepared to lead within the Transfer of Care Hub working alongside the discharge team supporting Hospital Home Care  with a strong emphasis on Governance issues, Patient Focus, Quality of Care and a desire to build and enhance a proactive team.

Working for our organisation

We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre. 

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services. 

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses. 

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

education

Essential criteria
  • 1st level registered nurse.
  • Recent post basic clinical qualification in relevant clinical area.
  • Mentoring / assessing qualification or Teaching qualification eg City & Guilds, Professional Trainers Certificate
Desirable criteria
  • recent leadership development

Skills, Ability, Knowledge

Essential criteria
  • Demonstrable appropriate/ specific clinical skills to designated area, e.g: • ILS / ALERT • Peripheral venous cannulation • Physical assessment skills • IV Administration • Venepuncture • ECG
  • Ability to prioritise and demonstrate time management and organisational skills in managing self and others
  • Demonstrate ability to obtain and evaluate information to aid decision making and to develop others in this area.
  • Ability to manage the care of a wide variety of patient conditions.
  • Understanding of clinical governance and its application.
  • Ability to develop standards of care.
  • Committed to CPD of self and others.
  • Demonstrate people management skills including: • Motivation of staff and team building • Negotiation • Decision Making • Leadership • Staff development • Conflict and problem solving
