Job overview

An exciting opportunity has arisen for a dynamic, organised and motivated person to join the Urology Cancer Team. This a new post for a Cancer Patient Navigator within the team, supporting the delivery of the Urology Cancer Pathways.  The successful applicant will assist the clinical team in proactively managing  rapid diagnostic service, providing holistic care to patients referred with a suspicion of or diagnosed with cancer.

Effective communication skills are essential; in conjunction with clinical colleagues, you will help patients and their families navigate the complex health care system. Experience in health or social care would be advantageous, with evidence of being a team player whilst having the ability to work on your own within the boundaries of the job description.

Main duties of the job

  • Tracking the progress of cancer patients through their pathway of care including through the diagnostic phase, pursuing results and ensuring that all results are managed appropriately to avoid potential delay. Patients may present through various routes and recording and tracking must be in accordance with National Cancer Waiting Times standards;
  • Ensuring all patients receive appointments and next steps in their pathway in a timely fashion whilst overseeing any other aspect of managing the patient pathway;
  • Maintain and update relevant databases as required.
  • To support patients and their carers re-access specialist services without delay should they need to do so

The post holder will work with the Urology Cancer team and supporting teams such as the Cancer Performance Team, specifically providing support to the Clinical Nurse Specialists,  Cancer Support Worker's,  MDT Coordinators and Cancer Trackers to ensure timely care for all patients in relevant specialities.

Working for our organisation

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Our community services for adults are provided in people’s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.

Education and Qualifications

Essential criteria
  • Good standard of general education including GCSE Maths and English or equivalent.
  • ECDL or recognised experience.
  • Evidence of ongoing personal development.

Experience

Essential criteria
  • Experience of undertaking a range of administrative duties.
  • Experience of using MS Office Systems.
  • Knowledge and use of medical terminology.
  • Word processing skills.
  • Knowledge of NHS data definitions relating to Cancer Standards and Cancer Waiting Times.
Desirable criteria
  • Previous NHS experience.
  • Experience of using a Patient Database.
  • Experience of working with spreadsheets and databases.
