Clinical Team Lead, Eden ICC

Job overview

Due to maternity leave an exciting opportunity has arisen for a Therapy Team Leader position over the Therapy Team and HCP Team,  within Eden ICC on a fixed term basis.

The skills of the post holder will be vitally important to continue to build relationships and networks within the ICCs. The post holder will be a clinician who can inspire, motivate and think differently in order to improve patient care with other key partners and stakeholders in the Eden ICC.

ICCs have three main aims:

  1. ICCs - the place (tackling the wider determinants of health though a population health approach)
  2. ICCs - the experience (more self-activation for better health outcomes through social prescribing and community engagement)
  3. ICCs - integrated teams. Each ICC has a coordination hub which coordinates the activity of each ICC. Clinical leadership for the hub is provided by a Professional of the Day who is either a registered nurse or therapist on a rota.  
    All ICC teams work closely together to ensure that patients can avoid going to hospital where health and care can be provided at home or in the community. In addition there is a focus on bringing people out of hospital as soon as they are medically fit.

Main duties of the job

Clinical responsibility, including overseeing complex patients; Supervision, teaching and training; staff management and leadership, service development and evaluation, quality and safety.

Person specification

Qualifications

Essential criteria
  • Qualified to degree level or equivalent experience
  • Evidence of post registration qualifications/ professional development in related health field
  • Royal College recognised Degree/Diploma in Occupational Therapy or • Chartered Society of Physiotherapy recognised Degree/Diploma in Physiotherapy or equivalent
  • Membership of relevant professional body
  • Registered with Health Professional Council
Desirable criteria
  • Post registration qualification in teaching/ assessing (ENB 998/NST 307)
  • Training and/or qualification relating to clinical supervision
  • Independent non-medical prescribing qualification
  • Post graduate teaching qualification
  • Leadership qualification

Experience

Essential criteria
  • Experience post qualification
  • Experience of providing supervision.
  • Demonstrable experience of providing teaching/training
Desirable criteria
  • Community Health Care rehabilitation

Knowledge
