Job overview

The Acute Referral Triage Nurse is a key role within the Urgent & Emergency Care Collaborative and is a fundamental part of the Multi Agency Telephone Coordination Hub (MATCH). 

MATCH is a newly implemented single point of access service for acute medical referrals. Based at Wigton Community Hospital, MATCH provides advice and expert decision making to referrals from a range of services within the healthcare economy of North Cumbria, including Primary Care, NWAS and NHS111.

The post holder will lead the triage and streaming process for all urgent and emergency care referrals into the Acute Trust.  The post holder will ensure patients are directed to the right place via the most appropriate clinical pathway, based on clinical assessment and referral information.

Main duties of the job

This role involves autonomous advanced decision-making, critical thinking, multidisciplinary collaboration, and service improvement initiatives. The Acute Referral Triage Nurse will be pivotal in optimising patient flow, improving clinical outcomes and supporting admission avoidance by ensuring we are safely and effectively managing the right patients in the right place. Strong leadership, advanced clinical assessment and triage skills and experience in acute care are essential.

Person specification

Qualifications

Essential criteria
  • • Registered Nurse with a current NMC registration
  • • Further postgraduate qualification relevant to speciality.
Desirable criteria
  • • Clinical assessment and history taking qualification.
  • • Leadership Qualification.
  • • Triage Qualification.

Knowledge & Experience

Essential criteria
  • • Significant post-registration experience, including substantial experience in urgent care, emergency care, or acute nursing.
  • • Evidence of continuous professional development (CPD) in relevant clinical areas.
  • • Understanding of national and local guidelines for triage and referral management.
  • • Previous experience in a triage or referral coordination role
  • • Professional knowledge acquired through advanced learning
  • • Evidence of management skills gained through experience
  • • High degree of IT literacy.
  • • Experience of mentoring and developing staff.
  • • Clinical experience as a Band 6 / Senior Staff Nurse level or above.
  • • Data collection and audit experience.
  • • Knowledge of clinical governance, risk management, local and national policies and patient safety principles.
  • • Ability to problem solve, analyse, compare and interpret complex information before making a decision
  • • Demonstrated ability to implement service improvements and manage change.
Desirable criteria
  • Experience in policy development or clinical guideline implementation.
