Job overview
The Derby and Derbyshire Care Transfer Hub will receive, process, coordinate and track intermediate care referrals for all Derbyshire residents.
The Hub Administrator role involves providing essential administrative support to the hub by overseeing and managing a team inbox, handling telephone communications, updating systems to support data collection and reporting, and assisting hub practitioners with various administrative tasks to ensure efficient workflow and effective service delivery.
Main duties of the job
The post holder will have experience within a customer service role, be friendly, approachable, empathetic and have excellent communication skills.
The hub administrator will function as the first point of contact and representative for the hub via telephone or email.
They will liaise with a wide range of stakeholder groups including but not limited to Derbyshire citizens and their families, clinical staff, care providers and support services. There will be an expectation to answer and refer queries, problem solve and be flexible to the changing needs of the service.
The post holder may be party to sensitive and confidential information of a personal, financial and political nature and will therefore be relied upon to observe and comply with specific and implied confidentiality rules and procedures.
To ensure the accurate and detailed recording and processing of statistical data to provide the evidence required by the Trust, Integrated Care Board and National reporting platforms for monitoring and evaluation purposes.

Essential criteria
- Educated to include 5 GCSEs grade C/Level 4 or equivalent. To include Maths and English
- Flexible to change and learn new IT skills to meet NHS changing service needs
Desirable criteria
- Competent in using IT systems including SystmOne (S1), Electronic Staff Record (ESR), Oracle, Microsoft Word, Outlook, Excel and Teams
