Job overview

This is a 12-month Senior Clinical Fellowship in Otology at University Hospitals of Derby and Burton NHS Foundation Trust (UHDB), one of the largest NHS Foundation Trusts in England, serving a combined catchment population of over 700,000 across Derbyshire and East Staffordshire.

The post is designed for surgeons who have completed or are nearing completion of specialist ENT training (FRCS ORL-HNS or equivalent) and wish to develop sub-specialist competency in otology, endoscopic ear surgery, and paediatric otology to a level appropriate for independent consultant practice. The fellowship is graded at ST8 equivalent.

This is a joint post spanning Royal Derby Hospital (RDH) and Queen's Hospital Burton (QHB), reflecting UHDB's integrated dual-site ENT service. The Fellow will work closely with all four substantive otology consultants and the paediatric ENT lead, gaining unparalleled breadth of surgical and clinical exposure.

Please note: This is a Trust post and is NOT recognised for training by the Royal College of Surgeons (RCS) or the East Midlands Healthcare Workforce Deanery.

Main duties of the job

The Fellow will be a senior non-consultant member of the ENT department, operating at ST8 equivalent level. Core clinical responsibilities include:

 Conduct and assist with otology theatre lists – endoscopic and microscopic ear surgery, mastoidectomy, stapes surgery – at both RDH and QHB, with graded autonomy under named consultant supervision

  Run dedicated Otology and Balance outpatient clinics at RDH with named consultant oversight

 Contribute to QHB Otology clinics as part of the joint Burton post commitment

 Participate in paediatric ENT and paediatric otology clinics and   theatre lists

   Cover the SPR on-call rota on a 1 in 8 'hot week' basis, supervising  emergency cases and ward patients during the working day

  Supervise and train junior doctors (FY2–ST3) in ENT assessment and surgical skills

Attend and contribute to the Head and Neck MDT and all departmental governance meetings

 Lead or co-lead at least one audit or quality improvement project suitable for publication or national presentation

  • Design, conduct, and write up research in collaboration with the consultant team

Detailed job description and main responsibilities

UHDB ENT and Head and Neck Surgery is a high-volume, consultant-led service operating across two major acute hospital sites:

   Royal Derby Hospital (RDH) – the principal surgical hub for complex otology, endoscopic ear surgery, head and neck oncology, rhinology, and thyroid and parathyroid surgery.

  Queen's Hospital Burton (QHB) – a busy District General Hospital site with a high-volume general ENT, paediatric ENT, and otology workload.

 

The department comprises 13 consultants covering head and neck surgery, thyroid and parathyroid surgery, rhinology, paediatric ENT, and otology. The Otology and Balance service is a particular strength of the department, underpinned by:

  A dedicated Endoscopic Ear Surgery (EES) programme – one of the most active in the East Midlands, with transcanal endoscopic approaches used routinely for cholesteatoma, tympanoplasty, ossiculoplasty, and stapedotomy.

 A busy mastoidectomy programme for both chronic suppurative otitis media (CSOM) and cholesteatoma, utilising both traditional microscopic and combined endoscopic-microscopic techniques.

     Active paediatric otology including paediatric cholesteatoma.

     Dedicated vestibular assessment and balance clinics conducted at RDH. Balance MDT every 2-3 months.

 Close links with regional audiology, cochlear implant referral network, and specialist ENT nursing teams.

Experience

Essential criteria
  • Minimum 5 years as a Type 1 (NTN/VTN) Specialist Registrar in ENT, or equivalent
  • Demonstrable operative experience in otology and middle ear surgery
  • Experience of, or training in, endoscopic ear surgery
Desirable criteria
  • Prior fellowship in otology
  • Experience in paediatric ENT and paediatric otology
  • Logbook evidence of ≥30 mastoidectomies as primary surgeon

Motivation

  • Demonstrates enthusiasm for endoscopic ear surgery

  • Ability to organise own learning and CPD independently

  • Temporal bone dissection or simulation teaching experience

Qualifications

  • MBBS /MBChB or equivalent qualification

  • Higher Degree (MD, PhD, or MS)

Audit and Academic Research

  • Evidence of active participation in audit
  • Evidence of research achievements e.g. publications of relevant study or laboratory and/or clinical research material in peer review journals, presentations at regional or national level
  • Evidence of participation in risk management
