Main Duties of the Job
This is a fantastic opportunity to provide high-quality, patient-centred physiotherapy across a range of community settings. You’ll work autonomously and as part of a dynamic multi-disciplinary team, which includes nurses, paramedics, occupational therapists, social workers, dietitians, and more.
Your day-to-day will include:
- Delivering high-quality, patient-centred physiotherapy to support rehabilitation and independence at home.
- Managing your own caseload across a wide range of conditions and levels of complexity.
- Working closely with GPs, Consultants, Nurses, Paramedics, Occupational Therapists, Social Workers, and other professionals.
- Preventing avoidable hospital admissions and facilitate timely, safe discharges.
- Participating in our supportive weekend and evening rota (no overnight on-call commitment).
- Engaging in internal rotation opportunities, including Urgent Community Response work, to diversify your clinical skills.

Detailed job description and main responsibilities
We are looking for a physiotherapist who is:
• HCPC-registered Physiotherapist with broad clinical experience, including MSK, neurology, respiratory and elderly care.
• Confident working independently and managing clinical risk, including lone working in the community.
• Committed to high standards of patient care and passionate about working in a dynamic community setting.
• An excellent communicator and team player, able to work collaboratively across a multidisciplinary team.
• An energetic, focused, high performer with initiative, who can remain calm under pressure and who enjoys a challenge.
• A driver with access to a vehicle for community visits.

Qualifications and Training
Essential criteria
- Diploma/degree in physiotherapy
- State registration – current HCPC certificate
- Evidence of CPD maintained in a portfolio, including attendance at recent Postgraduate courses relevant to the clinical field
- Experience within the NHS including experience as a junior in a broad rotational post covering a range of experience in orthopaedics, elderly care, medical, surgical, neurological and musculoskeletal conditions
Desirable criteria
- Post graduate training/experience in rehabilitation and elderly care
Knowledge and Skills
Essential criteria
- Working knowledge of a broad range of musculoskeletal, neurological and medical conditions common in the elderly
- Competent IT skills
- Knowledge of the relevant NSFs and clinical guidelines
- Skills and experience in rehabilitation of amputee patients
- Skills and experience in respiratory physiotherapy
Desirable criteria
- Experience of primary care rehabilitation
- Knowledge of the safeguarding adults process
- Experience of UCR and D2Ax
Special Experience
Essential criteria
- Experience of working as part of a multidisciplinary team.
- Experience supervising student physiotherapists or physiotherapy assistant/technical instructors/support workers.
- Some experience working with a biopsychosocial approach to assessment and treatment
- Experience of working in inpatient, outpatient and community settings.
- An understanding of Clinical Governance and its implications for services, including experience of quality issues and participation in research and audit
Desirable criteria
- Knowledge and understanding of complex legislation surrounding health and adult social care.
