Community Hospitals General Rehabilitation Units Therapy Lead

A fantastic opportunity has arisen for a proactive and motivated Clinical Therapy Lead to join the General Rehabilitation Unit for Community Hospitals at University Hospitals Plymouth NHS Trust. The post holder will be expected to develop, embed and lead the Therapy team in delivering new seven-day rehabilitation services.

The post holder will need to be highly experienced in hyper acute / acute rehabilitation and have proven leadership and service improvement skills. Excellent communication skills and a sound understanding of systems working and experience supporting the delivery of rehabilitation across South West Peninsula are critical to this post. The successful applicant will also provide essential rehabilitation support and leadership to the wider MDT.

 Potential candidates must be a HCPC registered Therapist (Dietician, Occupational Therapist, Physiotherapist or Speech and Language Therapist) and have recent experience of leadership, management and service improvement within the acute setting at a senior level.

To be responsible for managing and delivering patient centred intensive rehabilitation and reablement within the general rehabilitation units across UHP’s footprint inclusive of community hospitals.

As an experienced practitioner to be responsible for planning, co-ordinating, delivering and evaluating high quality Therapy service provision to general community hospitals patients and associated clinical areas, with input to future planning for care and management in conjunction with therapy, nursing medical and social care leads.

To be instrumental in facilitating patient flow, championing a Homefirst ethos and supporting patients to return home, to remain independent for longer and remain at home for longer through engagement with partner organisations. 

Manage a highly complex caseload incorporating patient assessment, diagnosis, treatment and discharge from care. Provide an expert resource and specialist advice to other health care staff/ patients/carers/other agencies.

Be a proactive member of the multidisciplinary team and will contribute to service development and training including audit/ general rehabilitation targets, and supervision of junior colleagues within the mixed MDT.

To support the Therapy Directorate Manager, Community Hospitals Managers and UHP Divisions / Directorates in the day-to-day management, coordination and development of clinical service delivery.

Clinical –

1.       To be an expert practitioner, particularly for patients with complex therapy needs.

2.       To lead assessment of referred patients requiring therapy. Liaising with medical and clinical staff on the treatment and care of patients referred.

3.       To provide specialist advice to other therapy team staff on the management of complex cases and ensure systems to monitor referrals within the acute service.

4.       To undertake assessment and care planning of routine and complex patients in a manner that respects people’s privacy dignity and individuality.

5.       To ensure a high standard of patient care is provided, developing, implementing and reviewing clinical standards relevant to work undertaken.

6.       To co-ordinate the clinical activity of the team to ensure provision of a responsive service that supports patient flow through the service/ organisation, including referral on to other services/ organisations as required

7.       To identify risks within the service, report accordingly and identify appropriate treatment plans to negate the risk.

8.       To be a core member of the MDT and actively participate in MDT meetings and ward rounds.

9.       To ensure effective two-way communication with patients, carers and other healthcare professionals regarding all aspects of patient care, supporting equality and respecting diversity at all times.

10.   To provide and receive complex information that requires empathy and reassurance, communicating where they might be barriers to understanding.

11.   To further develop links with all clinical specialties across the Trust promoting therapy in the management of disease, treatment and prevention of disease.

12.   To identify opportunities to extend the scope of practice and in particular the development and expansion of integrated working in clinical teams.

13.   To comply with local and Trust policy on documentation guidelines

14.   To record and submit clinical and professional activity according to department standards.

15.   To provide clinical advice to the as appropriate

16.   To contribute and lead on aspects of Service Transformation and service improvements as agreed with the Therapy Directorate Manager.

17.   To work collaboratively across the patients clinical care pathway with other clinical leads and Therapies Managers to ensure consistency of service delivery within the clinical streams.

Knowledge and Experience

  • Varied and evidenced post graduate demonstrable experience with broad clinical experience working at a senior/specialist level as a therapist (OT / Physiotherapy / Dietetic or Speech and Language) within an acute NHS rehabilitation services.
  • Proven evidence of CPD maintained in a portfolio including attendance at recent postgraduate courses relevant to the general rehabilitation clinical field.
  • Proven evidence of extensive clinical demonstrable experience of working to a specialist level in a broad range of specialist rehabilitation including experience of working with frailty, trauma and orthopaedic, cardiorespiratory, neurological, and cognitive difficulties.
  • Advanced skills in assessing and treating motor and sensory difficulties.
  • Current in-depth proven knowledge and understanding of national guidelines and standards in rehabilitation, enhanced supported discharge, hospital to home and HomeFirst principles.
  • Professional creditability/proven skill as a therapist within multidisciplinary teams at a senior level.
  • Proven Management and Leadership skills, including effective people management. Proven evidence of ability to organise effective education programmes and high skill level in clinical supervision.
  • Proven research / service improvement skills and proven evidence of recent audit activity.

Qualifications
