Job overview
The post holder is responsible for the Referral and Coordination on behalf of short-term services. The post holder needs to be able to co-ordinate detailed referrals from Health and Social Care Professionals, communicate and answer queries from all disciplines including Care Providers, members of the public and users of the service. To provide direct line management support and supervision of the support worker, ensuring strict compliance with Trust policies and procedures.A good general education, a relevant IT qualification or experience with the ability to use own initiative as well as being able to work within a team. Good organisational skills are essential.
Main duties of the job
·
· Responsibility for taking referrals and activating these services within the specified response time.
· To take referral information in a consistent manner, gathering additional information, arranging assessment, initiate and co-ordinate care with feedback to referrers.
· To review and co-ordinate needs of service users with professionals.
· To programme staff resources to meet the specified needs of the service users
· Deal with less complex referrals and forward to appropriate area, e.g. Social Services
· Patient Administration Systems – Link into as required to access patient details in relation to short term services.
· Produce statistical reports as required by Service Manager
· Working with a wide range of information- handling requirements on behalf of two or more agencies e.g. Torbay and South Devon Foundation Trust (TSDFT), Adult & Community Services (ACS), Devon Doctors on call services
· High levels of typing skills and IT Skills involving extensive experience of Microsoft Office including Word, Excel and Outlook
· Actively maintain positive working relationships with colleagues and within the multi-disciplinary team

Detailed job description and main responsibilities
Communication with other team members of Multi – Disciplinary Team, Health & Social Care Professionals, Members of the public. This involves a complete range of personnel on different levels of communication. This involves both confidential and sensitive complex communication
The postholder needs to be able to co-ordinate detailed Referrals from Health and Social Care professionals, communicate and answer queries from all disciplines. This involves organising own day to day activities, planning of both straightforward and more complex on-going referrals, together with looking in to the longer-term planning of care
Direct and Incidental contact is required with both patients and internal/external colleagues. Provision of programmes of care as required by the referrer
The postholder should work both individually and within the Team to contribute to service improvement. The Postholder to undergo monthly supervision and annual appraisals
· To be responsible for the management of support workers, including appraisal, annual leave, sickness absence, PDPs, work place risk assessments To manage the support workers annual leave and sickness absence, ensuring that the Service Manager is kept informed of any issues. To deal with the initial stages of grievance and disciplinary procedures. To participate in the recruitment and selection process, including assisting with shortlisting and take part in interview panels. To train and coach staff to ensure compliance with standard operating procedures. The postholder has responsibility for working without supervision, and should adhere to the Lone Working Policy

Qualifications and Training
Essential criteria
- NVQ3 Health and Social Care or other Equivalent
- Advanced level of keyboard skills
- Computer Literate with considerable experience of Word, Excel, and Outlook
- Good General level of education
Desirable criteria
- ECDL
- Diploma in Health & Social Care or equivalent
Knowledge and experience
Essential criteria
- Experience of working within a health or social care team environment
- Demonstrate skills in written and spoken English, adequate to enable the postholder to carry out the role effectively.
- Excellent interpersonal and communication skills
- Good organisational skills
