Job overview

TSDFT is looking to appoint a middle grade doctor at ST3 level, or above, to support our General Medical/Diabetes ward (Forrest Ward). There will also be opportunity for time in our specialty and sub-specialty clinics depending on experience. The diabetes specialty clinics include Type 1/2, insulin pump, antenatal, foot, young adult, transition and cystic fibrosis clinics. Specialty endocrine clinics include adrenal, thyroid, pituitary and lipids/hypertension. This is an exciting opportunity to get excellent experience in General Medicine and Diabetes/Endocrinology at TSDFT. You will be based on Forrest ward working closely with Consultants, other resident doctors, nursing and therapy staff. There are ample opportunities for work-based assessments from the supervising Consultants. You will also be allocated an Educational/Clinical Supervisor. Applicants will have completed 4 years postgraduate clinical work, be registered with the GMC and have experience working within the NHS.

Main duties of the job

• To build upon postgraduate clinical experience

• To develop further expertise in the identification and management of the sick patient

• To gain experience and familiarity in identifying and managing a wide variety of medical conditions, including making appropriate decisions regarding the need for admission, the development of strategies which might provide an alternative to admission and identifying where early discharge would be safe and appropriate

• To further develop skills of history taking, physical examination, appropriate investigation and rational prescribing

• To master a number of procedural techniques

• To hone communication skills with patients, relatives and colleagues

• To develop skills in managing time and conflicting priorities.

• To increasingly work towards independent practice at Registrar level, leading ward rounds, assessing patients, making decisions etc. with Consultant support and supervision available at all times

In addition, there are weekly Diabetes and Endocrinology MDT meetings and Junior Doctor teaching which you would be expected to attend and contribute to. Although this is not a training post it should be possible for an enthusiastic doctor to map their experiences to many of the core curriculum programme competences as all aspects of the syllabus are relevant to this attachment. We encourage our doctors to involve themselves in audit and research. You would be expected to make yourself familiar with the processes of the audit cycle and to assess the role of audit in clinical governance and in improving patient care. Supervision for projects will be available, and support may also be available from hospital audit departments.

Detailed job description and main responsibilities

Physical skills

·         Keyboard skills with appropriate adaptations and adjustments as required

 

 

Patient and client care

·         Practise in a senior trainee role (equivalent to registrar), with supervision from consultant colleagues, in all areas of general medicine including diabetes and endocrinology.

·         Provide timely and comprehensive assessment and treatment of people with complex health needs

·         Practise using a patient-centred approach, championing the patient’s wishes and goals when negotiating investigation and treatment plans.

·         Facilitate safe and timely patient discharges using a ‘Home First’ approach whenever able.

·         Work in a variety of settings, for example, general medical wards, general outpatient clinic.

·         Practise and promote a high standard of communication with patients, carers, and colleagues to include verbal and written communication.

·         Respect patients’ dignity, ensure confidentiality and work to overcome communication barriers.

·         Practise safe prescribing and deprescribing and safe use of medical devices.

·         Be aware of legal responsibilities, capacity assessment, consent framework, data protection and equality and diversity.

 

Policy and service development

·         Actively engage with continuing professional development and reflective practice.

·         Keep a portfolio of workplace-based assessments, reflections and evidence of competence.

·         Maintain and evidence professional knowledge of current best practice.

·         Comply with the Trust’s core mandatory training and identify personal development requirements/training through the appraisal process and personal development planning.

 

 

People management and training

·         Lead a team of junior doctors and multidisciplinary colleagues, be approachable and a role model.

·         Demonstrate an understanding of leadership styles and use this to create effective and successful team relationships.

·         Demonstrate the ability to appropriately challenge others, escalating concerns when necessary.

 

Research and development

·         Deliver regular teaching to junior and MDT colleagues.

·         Participate in our departmental medical students teaching programme.

·         Supervise junior colleagues and provide on the job training.

·         Be involved in assessing and providing formal or informal feedback for colleagues in medical and non-medical roles.
