Job overview

The Community Rehabilitation Team will support people to reach an optimum level of function; focussing on activities of daily living, vocation and leisure and the post holder will work towards these goals and:

Provide care for people both in the community and for those planning discharge from the Rehabilitation Inpatient Units with severe mental illness who have multiple and complex needs. This is under the direction of qualified practitioners.  

To provide support to an allocated group of service users, promoting their recovery and social inclusion and maintaining them in their community environment (STR role). 

To focus on the direct needs of service users, working across boundaries of care, organisation and role, coordinated through the assessment and planning and implementing their individualised My Wellbeing Plan.

Main duties of the job

You will offer intensive support for people requiring a recovery approach that is flexible according to need but without fostering dependence.

You will work with the trained staff to identify activities that the patient enjoys, finds meaningful and which occupy and divert them in a positive way.

You will ensure consistently high standards of record/case note recording, maintaining electronic and integrated records and case notes in accordance with the standards policy and relay verbal information where appropriate.

You will maintain confidentiality and security of information in all matters relating to patients. 

You will attend training courses and participating in personal development training as identified by the Team Leader and maintain responsibility for ensuring mandatory training is up to date.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
