Job overview
We are seeking an enthusiastic and experienced Band 6 Nurse to join our multidisciplinary team as a Frailty Practitioner. This is an exciting opportunity to play a key role in delivering high-quality, person-centred care for patients living with frailty, supporting early identification, comprehensive assessment, and proactive management.
The post holder will work across community settings, providing specialist clinical input to optimise independence, reduce hospital admissions, and improve patient outcomes. You will act as an autonomous practitioner while working collaboratively with GPs, therapists, social care partners, and wider services to deliver coordinated care.
You will contribute to the development of frailty pathways and support service improvement initiatives, ensuring care is evidence-based, safe, and responsive to the needs of the local population.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service can present a valid DBS certificate instead of requiring a new check.

- Evidence of recent professional development
- Experience of working in community
- Experience of managing change
Desirable criteria
- Physical examination qualification
- Experience of working with adults in long-term care facilities
- Experience at band 6 level, or above
- Able to use own initiative and appropriate risk taking
Job specific experience

- Experience of managing change
- Demonstrable knowledge of assessment and therapeutic interventions in area of specialism
Desirable criteria
- Experience at band 6 or above
- Recent previous experience within a comparable community role
Managerial/ Supervisory experience
