If so, we have an exciting opportunity for a dynamic registered specialist Nurse or Physiotherapist to join our highly specialised and committed respiratory clinicians, working as an integrated respiratory team, providing care to patients with Respiratory disease across North of Tees.
The post holder will be responsible for providing high standard of care to patients across Hartlepool and Stockton with regards to their respiratory health. The service provides care both in a clinic and home environment
it is essential that the postholder is a driver with access to a car
Main duties of the job
As a member of this team, you will be responsible for the pro-active management of patients who will need immediate specialist and comprehensive assessment and pathway based interventions. You will have direct links into the hospital and full support from a wider team including consultant.
This post will involve assessing and treating patients who have complex presentations or multiple pathologies, using advanced assessment techniques and adapting the patients care plan/treatment programme according to patients needs
The post holders will need to be able to work flexibly, including weekend working. Both full and part time hours will be considered. A full driving license is essential for the role

Detailed job description and main responsibilities
This is an ideal opportunity for a registered professional Nurse or Physiotherapist, who have experience and an interest in working within Respiratory, who are proactive, self-motivated and keen to be at the forefront of delivering high quality care, to join our team. The successful candidate will work closely with the Specialist Respiratory Practitioners in providing support and care for patients with Respiratory disease in the community and in patient setting. Recent training/qualification in relevant respiratory care is required for this post.
There will be an opportunity of development, through rotation into other Clinical roles, as a result of being a member of an innovative, responsive multidisciplinary team.
The integrated team is working in many innovative and exciting ways to improve quality of life for people with respiratory disease. The Respiratory team works within the community setting and includes Asthma, Pulmonary Rehabilitation and a home oxygen service. The service has implemented a "hospital at home" service, where more complex and intensive support is provided in the patients’ own home as an alternative to acute care for COPD and Bronchiectasis exacerbation management. The post will help the service to expand to other respiratory conditions. This service also supports early discharge from hospital. The post holder will be responsible for assessing and providing a treatment plan for their patients, PGDs are used within the service to allow appropriate treatment at the right time for the patient.
