We are looking for a motivated and compassionate clinician to join our Community Children’s Nursing Team. In this role, you will work in partnership with children, young people, and their families/carers to deliver highquality, evidencebased nursing care for a range of acute and shortterm conditions, following agreed care plans.

 

Working with minimal direct supervision, you will undertake delegated care and contribute to seamless, familycentred support.

 

The role requires close collaboration with the multidisciplinary team across both community and secondary care settings, ensuring a coordinated approach that places the child and family at the centre of all decisionmaking.

Prif ddyletswyddau'r swydd

Work in partnership with children, young people, their families/carers, and the Community Children’s Nursing (CCN) team to assess, plan, implement, and evaluate individual care needs, following agreed written care plans.

Communicate any changes in a child or young person’s condition promptly to the CCN and wider multidisciplinary team.

Collaborate effectively with all members of the multidisciplinary team to ensure coordinated and seamless care.

Utilise an empowerment and family‑centred approach to care, supporting families to be active partners and contributing to effective transition planning into adult services.

Provide a safe alternative to hospital admission and support early discharge when appropriate, ensuring robust risk assessment and risk management are consistently applied.

Promote the safeguarding and protection of children and young people by identifying and escalating concerns in line with Local Safeguarding Children Board and Trust policies and procedures.

Participate in clinical supervision and safeguarding supervision in accordance with Trust policy.

Demonstrate commitment to Improving Working Lives (IWL) principles and work flexibly to meet the needs of the service.

Communicate effectively with children, young people, families, and professionals, adapting communication methods to overcome barriers and ensure understanding.
