Job overview

The Development and Partnerships department at Sussex Community NHS Foundation Trust is looking for an enthusiastic and motivated Service Change Partner to support the Trust in delivering service improvements to ensure we deliver excellent care at the heart of the community. The post holder will support the design and delivery of an agreed programme of works, working with a range of professionals across the health and social care sector in Sussex. 

This is a hybrid role, requiring a combination of home working, attendance at the Brighton General Hospital office, and a visible presence within relevant clinical services as required. The post is full time or part time, with a minimum of 3 days per week for part time colleagues.

Main duties of the job

Service Change Partners work with clinical services to prepare services to respond effectively to required changes, quality improvements and our Trust's strategic objectives. 

Key requirements:

  • Strong influencer and communicator, able to flex style and gain trust with a range of differing stakeholders including head of service, clinical and operational service managers, clinical staff, commissioners and external providers. 
  • Ability to quickly learn and understand a range of different services and care pathways, maintaining a curiosity that provides a healthy  challenge to our partners. 
  • Strong governance, service change or project management skills to support design and delivery of service change.  

The post holder will provide expertise and leadership in driving projects forward alongside project sponsors and senior responsible owners. This will be done through: 

  • Developing and managing project plans and business cases within the departments portfolios as set by our executive committee. 
  • Working and influencing, as appropriate, a range of professionals across Trust and the Sussex health and care system to ensure best care and delivery. 
  • Analysing and evaluating a range of information to inform the service change proposals.
