In this varied and meaningful role, you’ll provide high‑quality clinical and educational support, help reduce health inequalities, and play a vital part in tackling HIV‑related stigma in the community. No two days are the same—whether you’re carrying out phlebotomy, supporting someone to manage their long‑term condition, delivering HIV awareness training, or collaborating with our multidisciplinary teams.

Main duties of the job
As a Community HIV Support Practitioner, you will:
Clinical & Patient-Facing Work
- Carry out phlebotomy and a range of delegated clinical tasks, including some solo community visits.
- Support personalised care plans, including medication adherence, self‑management and healthy lifestyle choices.
- Provide holistic support in partnership with HIV specialist nurses and other professionals.
- Escort or support service users to appointments when needed.
Communication, Education & Advocacy
- Build trusting, respectful relationships with people living with HIV.
- Communicate sensitively with service users, carers, and partner organisations.
- Help plan and deliver HIV education sessions across the Trust and wider community.
- Promote inclusion and challenge HIV‑related stigma.
Audit, Research & Administration
- Contribute to audits and research by collecting and recording data.
- Maintain accurate clinical documentation using electronic record systems.
- Support general administrative tasks essential to high‑quality service delivery.
Key Skills & Qualities We’re Looking For
- Strong communication and interpersonal skills

A full UK driving licence and ability to travel across Sussex are essential.

Detailed job description and main responsibilities
Clinical Responsibilities
- Deliver safe, person‑centred care in line with Trust policies and national guidelines.
- Perform phlebotomy and clinical tasks, adhering to infection prevention and control standards.
- Support implementation of care plans developed by registered clinicians.
- Identify and report safeguarding concerns in line with Trust policy.
Communication & Education
- Communicate effectively and professionally with service users, carers, colleagues and external partners.
- Assist with planning and delivering HIV education to staff, students and community groups.
- Promote HIV awareness and help reduce stigma across internal and external settings.
Service Delivery & Administration
- Maintain high‑quality electronic patient records.
- Undertake administrative duties essential to service functioning.
- Participate actively in team meetings, case discussions and service development forums.
Audit, Research & Quality
- Support research and audit by collecting and analysing relevant data.
- Contribute to quality improvement initiatives and implementation of research into practice.
- Uphold the Trust’s values of safe, effective and patient‑centred care.

Qualifications
Essential criteria
- NVQ Level 3 in Health and Social Care or equivalent or working towards
- Phlebotomy certification or working towards
Experience
Essential criteria
- Criteria Experience in a healthcare or community setting
- Experience of working with people with long-term conditions
Desirable criteria
- Experience working with people living with HIV
- Experience in audit or research
Skills and Knowledge
