Main duties of the job
Our Home First and Virtual Ward models are at the heart of this transformation—enabling people to recover in their own homes with hospital-level support, surrounded by comfort, dignity, and expert care.

You will be working independently but as part of a team, traveling to patients/ clients homes to deliver care.
You should have experience delivering care to patients/clients (nursing or social care setting) you will have a good standard of education which includes Maths and English GSCE (or the international equivalent)

Work pattern consists of 11.5 Hour shifts, with the service operating staggered shifts between 7am-10pm.

Essential criteria
- Good standard of general education, including English and Maths.
- National Vocational Qualification (NVQ) Level 3 or equivalent experience
- Satisfactory General Education and evidence of additional training relevant to role.
Experience
Essential criteria
- Previous experience in nursing or social care setting Customer care
- Experience of working independently as well as part of a team
Desirable criteria
- Previous community nursing and/or Physiotherapy, Occupational Therapy experience
- Experience of working in a community setting
