Job overview

A new opportunity has been developed for an Allied Health Professional (AHP) Duty Coordinator to work within Urgent Community Response team supporting across the Virtual Ward and Urgent Care pathways.

The Virtual Ward forms part of the Urgent Community Response (UCR) team. We are a 7-day service that supports acutely unwell patients that would otherwise be in a hospital bed, supporting both early discharge from hospital and admission avoidance.  The team responds to the needs of patients in the place they call home. The allied health professions within the team are Occupational Therapy, Physiotherapy, Dietetics and Speech and Language Therapy.

We require a dynamic individual to play a vital role in supporting Therapy teams to triage and prioritise referrals, completing wellbeing calls and support with urgent therapy visits for assessment, equipment provision and support rehabilitation of patients with Physiotherapy or Occupational Therapy needs. 

You should be confident, proactive and have excellent communication skills with a passion for patient care and drive, enthusiasm, and talent to develop services to support patient to remain at home where possible. 

Main duties of the job

The post holder will have previous experience in a role based in clinical settings. You will be part of a highly supportive team which uses an inter-professional model to support a broad range of patients.  Our EDGH and Conquest teams offer continuing professional development and clinical supervision.

As a Band 4 AHP Duty Co-ordinator you will: work as part of a team yet confident in working autonomously. Lead the triaging of referrals coming into the Therapy teams. Support the co-ordination of patient management by allocating visits and managing effective on/off boarding processes. Work flexibly and visit people in their own homes, conducting basic assessments and treatment plans, and supporting therapists with their patient caseload. 

The ability to travel around the county in is essential

Qualifications

Essential criteria
  • Good level of general education including A levels and GCSE or equivalents including functional skills level 2
  • An advanced / higher level apprenticeship for example Associate Practitioner or similar and / or NVQ level 4 in relevant subject or equivalent level of knowledge and experience
Desirable criteria
  • Care certificate

Experience

Essential criteria
  • Able to demonstrate substantial experience in health and care settings including in the relevant field of work.
  • Experience of computerised record systems
  • Evidence of interprofessional working

  • Ability to travel independently to community patients/between sites - UK driving licence
