Job overview

We have a great opportunity for a Senior Clinical Assessor to join our award-winning End of Life Hub Care Hub, known as ECHO. This is a unique collaboration with partners across Coastal West Sussex, including our two Local Hospices, the Acute Trust and a specialist Macmillan Team.
The ECHO Hub operates 365 days per year, 7 days a week, 24 hours a day, including Bank Holidays & weekends with shift patterns ranging from 06:45-14:45 & 11:15-19:15.









Main duties of the job

The aim of this service is to provide a responsive, dynamic, flexible partnership-based approach for patients registered to the service.

We are looking for people who have experience in community care, frailty or end of life care. Excellent clinical and communication skills are required, and a want to help make a difference to patients who are in their final stages of life. Palliative care experience would be an advantage.

You will be passionate about end of life care and have a good knowledge of frailty, with the ability to learn and develop as needed. Excellent organisational and communication skills are essential. You will be part of an expanding team based in Lancing, and the role will predominantly be one of telephone triage, liaison and coordination for patients registered or appropriate for ECHO.

Detailed job description and main responsibilities

To ensure all referrals are responded to in a safe and timely manner, to enable the patient to receive appropriate care within the community whenever it is safe to do so or to arrange appropriate admission if needs cannot be met within the community
 The post holder will be responsible for leading the shift and work collaboratively with One Call and will be expected to respond to any issues as they arise. They will also be expected to manage the call answering queues during their shift.
 The post holder will be expected to identify gaps in knowledge and to assist in the development of appropriate educational resources to support junior staff within the team.
The post holder does not have any financial responsibilities, including being a budget holder, a budget manager, responsibility for cash or valuables or responsibility for the maintenance of equipment, buildings or other physical assets.

Qualifications

Essential criteria
  • Registered General Nurse
  • Mentorship course or ENB 998 (teaching and assessing course) or must be willing to undertake within 1 year)
Desirable criteria
  • Educated to degree level or equivalent level of knowledge with significant post registration experience
  • Postgraduate qualification in palliative care, or prepared to work towards qualification

Experience

Essential criteria
  • Triage experience
  • Evidence of well-developed critical decision making skills
  • Ability to manage a team
  • Understanding of the Health Care Agenda
  • Demonstrates commitment to and recent evidence of further education and professional development
  • Experience of supervising, coaching, supporting and mentoring less experienced staff
  • Demonstrates a positive attitude towards change
  • Experience of clinical supervision
  • Knowledge of clinical governance
  • Evidence of experience of palliative and end of life care
Desirable criteria
  • Experience of unscheduled care
  • Evidence of working with a wide range of people at all levels including clients, carers, therapists, health and social care colleagues and other professionals in palliative and end of life care.
  • Understanding of multi-disciplinary working in the care of adults
  • Understanding of a single point of contact
