Job overview
The Home First Service provides clinical interventions (including intravenous therapy), by a team of Nurses, Paramedics, Physio and Occupational Therapists alongside reablement support following an assessment of needs, to support Pathway One discharges following a hospital admission.
Operating 365 days per year, the service receives referrals from a range of health and care professionals and triages, assesses, and delivers care to the patient over several days to ensure the patient can remain safely at home.
Main duties of the job
The postholder will be able to deliver high quality care in the community, working with patients towards rehabilitation, self-management and independence throughout the continuum of care. They will within participate in a team of colleagues supporting evidence-based care, facilitating staff learning and development. This post is an ideal opportunity for an experienced Nurse or paramedic who is keen to develop both their holistic assessment skills and knowledge of urgent community nursing.

Qualifications and/or Professional Registration
Essential criteria
- Degree level qualification or equivalent
- Registered Nurse – NMC compliant
Experience
Essential criteria
- Experience in a range of different clinical settings (placements or employment)
- Experience maintaining accurate clinical documentation
Desirable criteria
- As a community health provider, we provide care across 0-100+ years, Experience of working with children/young people, dependant on the service
Skills and knowledge
Essential criteria
- Understanding of safeguarding responsibilities for adults and children, and knowing when to seek support
- Basic computer skills or willing to achieve within an agreed period
- Understanding of infection prevention and control
- Understanding of confidentiality, information governance and data protection requirements
- Understanding of how to work as an effective member of the multidisciplinary team
- Ability to promote independence and choice in undertaking patient care
Desirable criteria
- Application of research-based practice
- Knowledge of current local policies informing community health and social care services
- Ability to use electronic patient record systems
Other requirements
Essential criteria
- Ability to travel across Trust locations, if required
- Valid driving license and access to a vehicle, if relevant to the role
- Commitment to ongoing professional development and reflective practice in line with NMC Code
