Advanced Practitioner
NHS AfC: Band 7
Humber Teaching NHS Foundation Trust are proud award winners of the HSJ Provider of the Year 2019
Job overview
We are looking for a flexible and confident registered practitioner to join our fast-paced, forward-thinking service. You will work directly with (often through care co-ordination), and indirectly support, individuals with complex emotional needs. The ideal candidate will be comfortable working within a psychological formulation and utilising the therapeutic relationship to support clients to achieve their goals, balancing therapeutic risk taking with robust risk management, incorporating this within their own work as well as supporting others to do the same.
This post can be offered as a maximum 0.8wte and a minimum of 0.6wte. For applicants who have additional training in psychological therapies relevant to the client group supported by the service, we would support splitting this role to provide both care co-ordination and provision of the psychological therapy for one day per week.
The service often supports individuals who may be diagnosable with a 'personality disorder', as well as individuals with neurodivergent diagnoses, or no diagnosis. One element of this post will be to work towards developing an effective support pathway with the Hull CMHT to support joint working and early identification of individuals who may benefit from additional direct or indirect assessment, formulation and intervention from CENS.
Main duties of the job
-To provide assessment, formulation and intervention (typically within the context of case management or care co-ordination) for individuals with complex emotional needs, often presenting with high risks to self and/or others
-To effectively liaise with the wider system around specific clients and the client group, to strengthen the support provided by others (e.g. supported living providers, inpatient services)
-To utilise clinical supervision to reflect on your work and relationships with clients, integrating knowledge derived from this into the work
-Undertaking complex assessments and formulations which incorporate relevant risks, and balancing intervening and risk management alongside therapeutic risk taking
