Older Adult Specialist Liaison Practitioner
NHS AfC: Band 6
Humber Teaching NHS Foundation Trust are proud award winners of the HSJ Provider of the Year 2019
Job overview
A rare opportunity has arisen for a Band 6 Specialist Practitioner (37.5 hours/week) in the well-established Mental Health Liaison Service for Older Adults. Operating 7 days a week from 9 to 5, weekend shifts are shared across the team.
The service supports Hull University Teaching Hospitals Trust by providing mental health assessments, interventions, and support to patients aged 65+ attending Hull Royal Infirmary’s Emergency Department or admitted to Hull Royal or Castle Hill Hospital. Presentations may include acute distress, self-harm, suicidal ideation, or organic/functional mental health issues. The team also advises ED and ward staff on management strategies.
Uniquely, the team developed one of England and Wales’ first ED streaming suites, offering a calmer space for mental health assessments. The successful candidate will assess and triage patients for this area and determine appropriate discharge pathways. Meeting 1-hour and 24-hour KPIs requires working efficiently under pressure.
Applicants must have strong knowledge of mental illness, risk management, dementia, and delirium, and be confident in high-risk situations. Key qualities include enthusiasm, adaptability, flexibility, and a commitment to high standards of care.
Main duties of the job
In this role you will;
- Offer a Mental Health and Risk assessment service to a needs led patient group presenting at the general hospital and ED due to acute distress, acts of deliberate self-harm or suicide, experiencing organic and/or functional presentations
- Be committed to the parity of esteem agenda, and work closely with both physical and mental health clinicians to ensure a seamless delivery of care.
- Provide a Mental Health consultation, liaison and advice service throughout the departments.
- Assist and supporting general hospital colleagues in making decisions about treatment and care in complex situations.
- Provide formal and informal education and case consultation to registered and unregistered staff.
- Provide a communication structure between secondary mental health services, primary care and non-statutory services, as a means of ensuring the highest possible standards of patient care.
We are supported by a Consultant Psychiatrist and Clinical Leads and we offer real commitment to your professional development.
Practitioners are expected to work autonomously, delivering mental health assessments and interventions, while also collaborating within a supportive, multidisciplinary team. We value shared expertise to foster learning and ensure the best, safest care for patients and their families.
