Job overview
An exciting opportunity has risen for a Health and Wellbeing Coach. This position will be to support the Swap to Stop Campaign on our Inpatient Units as well as our staff within the organisation. You will be required to travel to different trust locations but this will mostly be focused around the inpatient and forensic units. You will support people to develop their knowledge, skills and confidence in managing their health, to improve health outcomes and quality of life to support them making positive changes via coaching and through providing access to interventions such as self-management education, peer support and more. There will be a particular focus on going smoke free through utilising nicotine replacement therapy or vapes as quitting tools to stop smoking.
The successful candidate will be compassionate, reflective and self-aware and enjoy working with a wide range of people. They will have good communication skills and a firm belief that people have untapped resources within them, that can be developed by providing high quality, non-judgemental support. They will be proactive with a flexible attitude, keen to work as part of a team and committed to developing themselves in this skilled role.
Main duties of the job
- Provide 1-1 support for patients within the inpatient units and forensic units to no become dependant on tobacco and to encourage a healthy lifestyle.
- Manage and prioritise a caseload, in accordance with the needs and support required by individuals. Along with a strong awareness and understanding of when it’s appropriate or necessary to refer people back to other health professionals/agencies, because the person’s needs are beyond the scope of the role
- Conduct health and wellbeing tests including a finger prick test for cholesterol, blood pressure checks, height and weight measurements.
- Health and wellbeing discussions including smoking cessation, physical activity and alcohol intake
- Provide smoking cessation support to employees of Humber Teaching NHS FOundaiton Trust
- Signposting and onward referring where necessary
- Utilisation of the MECC principal
- Attend and contribute to team meetings and events as required
- Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role
- Participate in regular health coaching supervision and continual learning

Qualifications and Knowledge
Essential criteria
- An understanding of the biopsychosocial model of health and the social determinants of health
- Able to work within a biopsychosocial model, using a range of tools and techniques to enable and support people, such as agenda setting, goal setting, problem solving
- Ability to travel across multiple sites
- Commitment to continuous learning and development.
Experience
Essential criteria
- Demonstrable skills in supporting behaviour change
- Excellent group and one-to-one facilitation skills including conflict resolution
- A good understanding of physical to Foundation Degree equivalent to diploma level including health promotion, and mental health including policy/legislation
Desirable criteria
- Experience of working in a multidisciplinary team
- Experience of working in health and social care care/community development setting or similar Networking experience
Skills and Competencies
Essential criteria
- Proficient in Microsoft Office and web-based services
- Ability to work with minimal supervision and act decisively and ask for help when needed
- Requirement of frequent concentration e.g. implementing care plans
- Occasionally the work pattern may be unpredictable
- Occasional exposure to emotional or distressing circumstances
Desirable criteria
- Coaching/counselling qualification/ experience or other relevant qualification/experience involving reflective listening skills.
- Relevant training and experience in Self-Management Education (SME)
